1. Please give your age, sex and on a scale of 1-10 (one being lowest and ten being highest) where you would rate your sex drive?

2. if you are in a relationship (or have recently been in one) how did your sex drive compare to your partner's?

3. if you have ever expereinced an extremely low sex drive (or and extremely high sex drive) did you take any suppliments or medications to help you with the situation? If so, what did you take?

4. if you have expereinced what you would consider to be problems with your sex drive (either high or low) would you say those problems were more a result of a hormonal imbalance or a psychological one? Did you ever seek medical help for your "problem"?
