Get him a cake. Everyone loves one of those. And I dunno, a music CD? You can probably afford that.

I have to add - this is sweet. Your brother has a caring little sister.

20 is NOT old damnit! Anyway bake him a cake and decorate it all pretty like. Then throw down with some candles and surprise him with it. I promise he'll love it!

does he have a pc or an xbox or something? maybe a game? not an expensive one. you can find some good cheap games if you know where to look.

Don't get him clothes. I get them every year, and I let a big sigh each time. It's never a surprise.

Oh yeah I also think that it's very sweet that you want to do something for him. Another idea is you could write him a poem or something. My baby sister is very talented in that department (she's been published) and she writes me a poem every year on my b-day. They always make me smile

Take him out to see a movie, go to the zoo, to a carnival, those huge arcade places, or to a restaurant, even.

fuck, i'd love someone to take me to an awesome arcade and pay for it. there's only one here! and it's rubbish! i'm gonna make one.

My brother took me to a huge arcade place called Galaxy World when I was a kid. It was fun. I wish we could share those moments again, but he's way too busy now with his job and family.

If he is going out clubbing on the night of his birthday with his friends, here's what you do: get some paper and coloured pens/pencils and make him up a fancy 'coupon' for breakfast in bed the next day. Find out what he would like and when he would like it served and voila.

Better yet get him a box of condoms for that night with a bow and lil note attached saying "Remember to wrap that shit up! Happy Birthday!"
