How do you handle a girl who was once pretty, but is now descent?

Last saw her Spring of 2006 and she was a good 8/10 but her boyfriend was an apartment roommate.

Saw her a few days ago and she was a good 5.5/10 mainly because she gained weight and lopped off her hair, but she's got potential, from what I saw a year ago, and we hit it off the other day.

Oh and she broke up with the boyfriend, who turned out to be a huge douchebag and moved out a while back. She moved back to the area so I have an in to see her again and make my move if I decide to.

The question now is, how would I go about
a.) getting her to be an 8 again
b.) hitting that before someone else does.

Relationship is an option and I could just weather the storm, but I have no guarantees shes going to change. Opinions?
