From what I've heard from other artists (That have gotten these kind of requests on their website and then replied) it seems that shows are booked by the guys owning the venue, not by the performers.

So it's more like the guys get invites from a bunch of venues rather than they sit down and say: Err, let's go to Seattle next!

Though this may not apply to all artists I guess.

It's also usually decided WAAAAY ahead of time. I know I saw Beasties on the show list for this Red Rocks season WAAAAAY before it was announced. Probably sometimes around april/may. (I wasn't gonna risk outing the show since technically I don't think I shoulda saw the confidental list) How it works at red rocks I think is that people place holds on certain days then confirm. So basically there is a concert on hold for everyday but not everyone confirms they want that day. Not sure how other venues work tho.
