i know it to be a mid-western thing. in my experience, people who say that generally dont give an eff about whomever they're blessing. i think its a cheap phrase that people use to broadcast that they are a good person while putting someone else down.

I have said it, to other adults in regard to kids that I work with. For example, there was one lad I was working with on a project about sustainable development recently, and there was this one concept that I kept going over and over with him, but he just wasn't getting it, so I was talking to his team leader and said something like "bless his little heart, it's not for lack of trying, he works really hard, but he's just not there yet".

You can say anything, as long as you put "bless their heart" somewhere in there, and not be insulting.
